I’ve been researching microneedling and radiofrequency technology for many years and have seen results along the way, but as a scientist and plastic surgeon, I’ve discovered that advances in technology and seeing improved results are what motivates me to investigate the latest innovations for my patients. This procedure, in my opinion, represents the next generation of radiofrequency (RF) microneedling. What’s most intriguing is that excellent outcomes can be obtained without the pain and discomfort that is often associated with this type of treatment.
The treatment has various cosmetic applications, but I particularly enjoy using it to treat aging skin, skin laxity, pigmentation, and vascular lesions. I’ve also had great success with acne scars, rosacea, face pigmentation, fine wrinkles, and other lesions.
How does SYLFIRM X Work?
The treatment combines radiofrequency energy with tiny microneedles that deliver energy into the skin. The device is strategically adjustable, in that the needle depth and power can be dialed up or down for maximum results.
What sets this procedure apart is the pulse wave setting. It is the only fractional RF Microneedling device featuring both a pulse wave PW (short pulse) and continuous wave (CW) mode, in the world today. Other RF microneedling devices have a continuous waveform capability only.
Different settings are recommended for each situation. Pulse wave (PW) is usually recommended for pigment and redness and small broken blood vessels. Continuous wave (CW) is recommended for skin tightening and scarring.
Because of the speed of treatment and the ability to use both short pulse in PW mode as well as long pulse CW mode to address deeper structures, we have seen great results for skin texture, wrinkle improvement, and pigmentation. It is also my go-to device for hand rejuvenation.

Who is a good candidate?
Because the SYLFIRM X system has such adaptability, it can treat a wide range of skin issues. This ranges from basic skin tightening in the dermis to the treatment of pigmentation on the outer epidermis. SYLFIRM X is an FDA-cleared RF microneedling system that can be used to treat these skin issues:

Who is not a good candidate? Because SYLFIRM X is so versatile it can be used for most patients, however there are some patients with certain conditions that preclude them from having this type of treatment. Such as:
- Are on Accutane for acne
- Have an active cold sore breakout
- Have a skin infection
- Have active psoriasis or eczema
- Are receiving chemotherapy
- Are receiving radiation therapy
- Are pregnant
- Are taking anticoagulants
- Have a history of keloid or hypertrophic scarring
How Many Sessions Are Recommended?
Depending on the condition of your skin, I recommend a series of three to four treatments. My staff and I find that we can reduce the downtime for patients and enhance their results by using an Exosome topical post procedure. This reduces erythema (superficial reddening of the skin) and promotes healing time.
Patients find the procedure comfortable and are very excited with the results.
This procedure was developed to target pigmentation and skin rejuvenation for all skin types, with reduced treatment time, and minimal discomfort or downtime.
With this treatment we can treat both the face and body which patients have really appreciated due to how little time is required to get the results they are looking.

Most Frequent Questions And Answers
For most patients they see visible tightening results immediately, as the collagen transforms in response to the microneedles and RF energy. However, the greater results appear as the skin initiates production of new collagen to send to the treatment areas. Most patients feel they see continual improvements over about 4 weeks. As I follow patients, I have observed improvements continue to build for six months to a year.
Skin rejuvenation through the RF microneedling provides permanent increases in collagen in the dermis, but you must remember that with each passing year our bodies produce less and less collagen. So, the increases we initiate basically only stem the tide. That makes it a good idea to continue SYLFIRM X treatments as an ongoing maintenance treatment twice a year.
For issues such as age spots or rosacea, a single treatment may be all you feel you need. However, what I find with most patients is repeat treatments can expand their results.
For optimal results over time, I recommend at least bi-annual treatments.
Due to the technological advances of SYLFIRM X, the pain and discomfort normally associated with Microneedling and Radio Frequency treatments are so minor, most patients can tolerate treatments with little or no numbing. However, we are sticklers for patient comfort and to be proactively sensitive to our patient’s needs we apply topical numbing cream prior to your SYLFIRM X RF Microneedling session.
Patients describe feeling a slight warming sensation as the RF energy enters the dermis, but once again they do not find it to be painful.
Most patients tell me after their treatment is finished their skin feels a little tight and slightly tender. We suggest applying an cool compress to the treated area to settle things down and then I use an exosome topical post procedure to expedite healing.
Lastly and of great importance is to use sunscreen with SPF 30+ and sunscreen clothing. Remember, sun exposure is your skin’s greatest enemy.
Things to avoid: Harsh cleansers and scrubbing for a few days and stop using exfoliants for the same period. If you can avoid applying makeup for 24 hours that is recommended.
The technical explanation is SYLFIRM X has a wide spectrum of adjustable depths from 300microns to 4 mm. The 300-micron depth can address more superficial structures (diffused pigment, erythema) included the basement membrane which plays a key role in the aging process.
Other systems may require a different tip to address the shorter depth. The SYLFIRM X adjustable tip can be used from 300micron to 4mm resulting in an efficient and cost-effective treatment.
Why this information may be helpful to patients is due to the handpiece design and robotic delivery of the needles, the SYLFIRM X delivers a more comfortable treatment than what we have experienced with our other devices and the procedure is well tolerated by our patients.
The Pulse Wave mode provides shorter pulse duration to target smaller structures like pigmentation or some veins, while the Continuous wave mode is ideal for bulk heating and collagen remodeling
